By using the services of Shake It Off Dog Training, you acknowledge having read, to agree to, and to accept the following Terms & Conditions:

1. Shake It Off Dog Training will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training, training instruction, and care. However, to the extent that Shake It Off Dog Training is insured for any unintentional or negligent errors, omissions, or incorrect assertions, Shake It Off Dog Training will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by Shake It Off Dog Training and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others. I represent and warrant that I have provided Shake It Off Dog Training with full and complete, accurate information regarding any bite history and similar incidents or hazardous tendencies of my dog, and that I will update that information if it changes. I understand that I am and will remain responsible for the actions of my dog at all times. I hereby agree to indemnify, release, and hold harmless Shake It Off Dog Training, it’s owner, trainers, and any representatives, of any and all claims, whether the claims are made by myself, my family member, accompanying guests, or any third party, of any injury, expenses, costs, or damages to myself, my dog, or to any handler sponsored by me, that are caused by my dog. I agree that I will defend and indemnify Camille Lina or any representative of Shake It Off Dog Training for any injury, expense, costs or damages to any persons or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog.

2. I understand that dogs and puppies that are not fully vaccinated are at higher risk for certain diseases, and that although Shake It Off Dog Training considers the risk of disease during training, I hereby agree to indemnify and hold harmless Shake It Off Dog Training of any and all claims regarding my dog’s health. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service. This contract, together with language expressly incorporated into it in writing, is the full and complete agreement between me and Shake It Off Dog Training. A complete and accurate copy of this contract is as valid as the original. I further understand and agree that any behavior or health problems that develop with my dog(s) while at Shake It Off Dog Training will be handled and treated as deemed best by the staff, in their sole discretion. I expressly agree to assume full financial responsibility for any and all expenses arising or relating thereto.

3. If my dog(s) become ill or injured, Shake It Off Dog Training will attempt to notify me or the Emergency Contact using the phone numbers that are provided above. If neither myself nor Emergency Contact can be reached, Shake It Off Dog Training, at its sole discretion, may engage the services of a veterinarian and/or administer medicine or give other requisite attention to my dog(s) and the expense thereof shall be paid for by me. I authorize emergency medical care to be provided for my dog(s) by my veterinarian, or an appropriate alternate to be determined by Shake It Off Dog Training in the event my regular veterinarian is not available or that closer care is required. I will reimburse Shake It Off dog Training for any charges related to emergency care, including office visits, procedures, medications, surgeries, etc. I agree to indemnify and hold harmless Shake It Off Dog Training for all and any results thereof.

4. Day Training: I authorize Shake It Off Dog Training to enter my home during agreed upon days and hours for the purpose of training my dog.

5. Off Property Excursions: I authorize Shake It Off Dog Training to take my dog off my property during the agreed upon days and hours for the purpose of training my dog.

6. Owner Commitment: I understand that Shake It Off Dog Training is committed to helping dogs and their owners through the teaching and employment of science-based training techniques and principles. These techniques require commitment and consistency from all members of the family in order for the dog to be successful. Training is a full-time commitment for the entire course of a dog’s life. As the owner, I understand that my involvement and commitment to this process is critical to the success of any training sessions that occur. Shake It Off Dog Training reserves the right to terminate this contract if the owner(s) are not cooperating with and/or implementing the suggested advice and practice.

7. Payment Policy: Payment for Behavior Consultations, Training Packages, and Individual Training Sessions are due paid in full at the time of booking, prior to any services performed.

8. Cancellation & Refund Policy: We kindly require at least 24 hours notice to reschedule or cancel a Training Session or Consultation. Credit will be applied to your next session when changes are made 24 hours in advance of the scheduled appointment. Late cancellations, made less than 24 hours in advance, will be charged in full (or if you have purchased a training package, credit for that session will be deducted from your package) for the cancelled session. Shake It Off Dog Training will refund the cost of the cancelled session in accordance with the above terms, minus any credit card processing fees if payment was made using a credit card.

9. Training Packages: Please note that training packages expire 3 months after the first scheduled training session; once a training package has started it must be completed within 3 months. Exceptions may be made at the discretion of your trainer. Training Packages and Group Class purchases are non-refundable and non-transferable.

10. Gift Certificates: Gift Certificates are non-transferable and non-refundable. Gift Certificates do not expire. All training sessions must be scheduled before the Gift Certificate expires.

11. COVID-19 Policy: I warrant and represent that I will remain aware of and abide by local law and public guidance on reducing COVID-19 exposure, and will follow best practices in relation to Shake It Off Dog Training’s services regarding social distancing and sanitation. I understand that I may be asked to confirm my health and the health of all other members of my household, including awareness of any fevers or other symptoms, before each in-person training session. I represent and warrant that I will provide full and complete, accurate information as known to me.
